To the Dartmouth community, 

Dartmouth is hosting a COVID-19 vaccine clinic for Dartmouth students, faculty, and staff on June 28 from 9 a.m. until 7 p.m. at Alumni Hall in the Hopkins Center. This clinic will have Pfizer-BioNTech and Moderna vaccines available for those who would like to receive a primary series vaccine, a first booster, or a second booster. 

For information on your COVID-19 vaccine eligibility, review the U.S. Centers for Disease Control and Prevention’s vaccine guidance. Students can also contact the Dartmouth College Health Service, and faculty and staff can contact their primary care provider. 

Registering ahead of time is recommended; however, walk-ins are welcome. Please plan for the process to take about 30 minutes longer if you do not pre-register.

To register:

  • If you have not attended an Axiom-managed vaccine clinic in the past, begin by registering on Axiom Medical’s Vaccine Vault portal. If you have attended an Axiom-managed vaccine clinic, log in to your account with Axiom.

  • Follow the instructions. When prompted to enter an employee ID, enter your Dartmouth NetID.

  • If you received a vaccine other than Pfizer-BioNTech, Moderna, or Johnson & Johnson’s Janssen for your primary series, you must choose Pfizer-BioNTech for your booster.

  • If you are under 18 years of age and are getting a booster, you must choose Pfizer-BioNTech for your booster. On the day of the clinic, you must be accompanied by a parent or guardian who will sign an additional consent form on your behalf when checking in.

  • If you select “yes” when asked if you have allergic reactions, you will be prohibited from selecting an appointment. Email Axiom at vaccineservice@axiomllc.com to talk through whether you are an appropriate candidate for the vaccine you are requesting.

  • If you experience issues registering for the clinic, read Axiom’s instructions on how to register. You can also email Axiom at vaccineservice@axiomllc.com.

On the day of the clinic:

  • Bring your vaccination card to document your vaccination.

From Lebanon Street: Take the stairs or elevator across the hall from the Strauss Gallery up one level. Alumni Hall entrance is on your right.

From East Wheelock Street: Go up the stairs in the Moore Theater Lobby and follow around the top level of the theater, keeping right. After the restrooms, Alumni Hall is down a short flight of steps on the left side.

At this time, Dartmouth requires students, faculty, and staff to receive all recommended doses in their primary series of COVID-19 vaccine, and one booster dose when eligible, or have an approved exemption. If you receive a second booster, you may choose to submit your vaccination record to Dartmouth. Students can submit their record to the Health Service web portal, and employees can submit their record using the vaccine tracker form.

If you cannot attend this clinic but would still like a COVID-19 vaccine, same-day appointments are typically available at Alice Peck Day Memorial Hospital in Lebanon. You can schedule an appointment through a myD-H account or call 603-442-5612. Find more local COVID-19 vaccination options, including pharmacies and state-managed clinics, by searching vaccines.gov or calling 1-800-232-0233.
