To all students, faculty, and staff,
As a reminder, Dartmouth is hosting on-campus COVID-19 booster clinics from 9 a.m. to 6 p.m. on Monday, Jan. 10 and Tuesday, Jan. 11, at Alumni Hall in the Hopkins Center. Appointments are still available.
Here’s how to register:
Go to the Axiom Medical Vaccine Vault Portal.
Complete all required fields on the registration page and select “Register.” Students: use your NetID in the “Employee ID” field.
You will receive an email from noreply@salesforce.com on behalf of Axiom Medical Vaccine Vault <vaccineservice@axiomllc.com> with a link to schedule your booster.
When you select the link, you will be asked to change your password and select the booster shot you would like to receive. You will be directed through a series of screening questions, followed by a consent form. To receive your booster, you will need to answer each question and consent to the form.
The clinic will offer both the Pfizer-BioNTech and Moderna boosters.
Students: note that this consent form uses employee-specific language; there will be an additional consent form to sign at the clinic.
Select your desired appointment time and confirm your selection.

Eligibility and choosing your booster
As a reminder, all students, faculty, and staff are required to receive a COVID-19 booster by Jan. 31, or within 30 days of becoming eligible. Consult the CDC booster eligibility guidance for the most current information on when you are eligible for a booster and which booster shot you should receive.
Additional booster clinic
If you cannot attend the Dartmouth clinics, consider visiting the walk-in clinic at the former JCPenney location in West Lebanon, held on Tuesdays and Thursdays in January, from 11 a.m. to 6 p.m.
