To all faculty, students, and staff,
Dartmouth is hosting on-campus COVID-19 booster clinics from 9 a.m. to 6 p.m. on Monday, Jan. 10, and Tuesday, Jan. 11, at Alumni Hall in the Hopkins Center.
Here’s how to register:
Go to the Axiom Medical Vaccine Vault Portal.
Complete all required fields on the registration page and select “Register.” Students: use your NetID in the “Employee ID” field.
You will receive an email from noreply@salesforce.com on behalf of Axiom Medical Vaccine Vault <vaccineservice@axiomllc.com> with a link to schedule your booster.
When you select the link, you will be asked to change your password and select the booster shot you would like to receive. You will be directed through a series of screening questions, followed by a consent form. To receive your booster, you will need to answer each question and consent to the form.
The clinic will offer both the Pfizer-BioNTech and Moderna boosters.
Students: note that this consent form uses employee-specific language; there will be an additional consent form to sign upon arrival at the clinic.
Select your desired appointment time and confirm your selection.
Eligibility and choosing your booster
As a reminder, all students, faculty, and staff are required to receive a COVID-19 booster by Jan. 31, or within 30 days of becoming eligible. Consult the CDC’s booster eligibility guidance for the most updated information on when you are eligible for a booster and which booster shot you should receive. As of today:
Those who received the Pfizer-BioNTech vaccine are eligible to receive a booster five months after the second dose of their initial vaccination series. (This is a change from six months, the previously communicated eligibility period, and we plan to follow the five-month CDC guidance.)
Those who received the Moderna vaccine are eligible to receive a booster six months after the second dose of their initial vaccination series.
Those who received the Johnson & Johnson vaccine are eligible to receive a booster two months after their initial dose.
Those who received a COVID-19 vaccine approved for emergency use listing by the WHO must receive a Pfizer-BioNTech booster shot and are eligible for a booster six months after completing their initial vaccination series.
Those who are under 18 years of age can only receive the Pfizer-BioNTech booster.
Day-of-clinic instructions
To get to Alumni Hall from Lebanon Street, take the stairs or elevator across from the Strauss Gallery up one level. Alumni Hall is on the right. From East Wheelock Street, take the stairs to the Top of the Hop and follow the hall at the back of the room. Alumni Hall is down a short flight of steps past the restrooms.
Bring your vaccination card to Alumni Hall to be able to document your booster.
After receiving your booster, students should send their NetID and a photo of their vaccination card to medical.records@dartmouth.edu.
Employees, watch for a message on how to submit booster documentation.
Only Dartmouth-affiliated students and employees will be able to attend the booster clinics.
Exemptions from booster requirement
Individuals who have been approved for a medical or religious exemption from the COVID-19 vaccination requirement will be contacted alerting them if they need to reapply for an exemption or if their original request will suffice for this additional requirement. For individuals seeking an exemption from the booster requirement, medical exemptions require a letter from the individual’s primary care physician, and religious exemptions require a letter written by the individual.
Employees seeking an exemption can follow instructions on the Office of Institutional Diversity and Equity website.
Students seeking an exemption should send letters to medical.records@dartmouth.edu.
Other booster clinics
If you cannot attend the Dartmouth clinics, consider one of the clinics in the area:
Walk-in clinic at former JCPenney location in West Lebanon: Tuesdays and Thursdays in January, 11 a.m. to 6 p.m.
State of New Hampshire Booster Blitz: Saturday, Jan. 8, 10 a.m. to 3 p.m., at sites around the state.
COVID-19 winter updates
All on-campus dining services will be grab-and-go for the first two weeks of January.
All College-sponsored indoor social gatherings for the first two weeks of January are canceled as we watch the progress of the virus. Academic-related and religious events will continue as scheduled. We make this decision to preserve in-person instruction and minimize risk to our community.
Individuals who test positive for COVID-19 will be required to isolate for a minimum of five days and will be released from isolation upon receipt of a negative rapid antigen test result on Day 5 or 7, or after a full 10-day isolation.
Students who tested positive for COVID-19 over break should email documentation of their positive test results to medical.records@dartmouth.edu, followed by documentation of a negative antigen test if released from isolation in fewer than 10 days.
All students, regardless of where they spent winter break, are required to conduct a PCR test in-person at West Gym within 48 hours of arrival at Dartmouth. Arrival tests must be completed no later than today, Jan. 5. Students who will be arriving to campus late and cannot complete arrival testing by Jan. 5 should complete this form.
Given the high transmissibility of the omicron variant, we strongly encourage the use of medical-grade masks and snug-fitting multi-ply masks, as early data show they reduce transmission of the virus more than cloth masks do.
Students may expect to see some courses go online for some periods of time. If a large number of students in a course are out due to isolation or travel delays, the instructor may need to hold class online until students are able to return to class. We recognize that the situation is fluid and students and faculty will need to remain flexible as the term proceeds.
