Dear faculty and staff,

Dartmouth will be hosting another COVID-19 booster clinic on Monday, Jan. 31 from 9 a.m. to 8 p.m. at Alumni Hall in the Hopkins Center. This clinic will be open to students, faculty, and staff, as well as family members and housemates. The clinic will be open to adults (ages 18 and older) and minors (ages 12-17). Every participant must be registered online in order to receive a vaccination. For Dartmouth students, faculty, and staff, this clinic will be the last opportunity to get boosted at Dartmouth before the Jan. 31 booster submission deadline.

If you have already received your booster shot, please submit your booster record using this vaccine tracker form by Jan. 31. This process should take less than two minutes. If you previously submitted your initial vaccination information, you will only need to submit your booster record through the vaccine tracker form. Faculty and staff who received the booster at previous Dartmouth clinics or plan to receive it at the clinic on Jan. 31 must also submit their booster record using the form

According to Centers for Disease Control and Prevention (CDC) guidance, individuals recently infected with COVID-19 who are eligible for the booster can still receive the booster as long as they feel well.

Here’s how to register for the upcoming clinic:

  • Visit the Axiom Medical Vaccine Vault Portal.

  • Complete all required fields on the registration page and select “Register.” Note: Use your NetID in the “Employee ID” field. Individuals who do not have a NetID should leave this field blank.

  • You will receive an email from noreply@salesforce.com on behalf of Axiom Medical Vaccine Vault vaccineservice@axiomllc.com with a link to schedule your booster appointment.

  • When you select the link, you will be asked to change your password and select which booster you would like to receive (Pfizer-BioNTech or Moderna). Then, you will be directed through a series of screening questions, followed by a consent form. To receive your booster shot, you will need to answer each question and consent to the form.

  • Please select your desired appointment time and confirm your selection.

  • Note: If registering for someone else, each participant must register with a separate email address in order to be able to select an appointment time. If you are registering yourself and need to register a minor who does not have their own email address, we suggest you use your Dartmouth email address to register yourself and a personal email address to register the minor.

  • Note: All minors (ages 12 to 17) must be accompanied by an adult at the clinic and will need parental consent in order to be vaccinated. When registering a minor for this clinic, you will have an additional consent form that will need to be signed on the child’s behalf at the time of check-in at the clinic.

Axiom will not be able to provide boosters to individuals with the following conditions at this clinic:

  • Pregnancy/breastfeeding

  • Recent receipt of monoclonal antibody treatment

  • Currently taking medications or immunosuppressant drugs

  • Weakened immune system

  • Allergic reaction to other vaccine, to previous COVID-19 vaccine, or to polyethylene glycol

  • Recent dermal fillers

  • Certain medical conditions

  • A positive COVID-19 test result between registration and vaccination day

  • Note: If you select “yes” when you are asked if you have allergic reactions to anything, you will be prohibited from selecting an appointment time. In order to unflag your account, email VaccineService@axiomllc.com so the team can determine whether you are an appropriate candidate for the booster vaccination you are requesting.

Day-of-Clinic Instructions

  • Please bring your vaccination card to Alumni Hall to be able to document your booster shot.

  • After receiving your booster shot, please submit your booster record via the vaccine tracker form as soon as possible, as Jan. 31 is the booster requirement deadline.

  • Directions to Alumni Hall: From Lebanon Street: Take the stairs or elevator across the hall from the Strauss Gallery up one level. Alumni Hall entrance is on your right. From East Wheelock Street: Go up the stairs in the Moore Theater Lobby and follow around the top level of the theater, keeping right. After the restrooms, Alumni Hall is down a short flight of steps on the left side.

Eligibility and Choosing your Booster

Please consult the CDC booster eligibility guidance for the most updated information on when you are eligible for a booster and which booster you should receive. 

Exemptions from the Booster Requirement 

Individuals who were previously approved for a medical or religious exemption from the COVID-19 vaccination requirement will be contacted if they need to reapply for an exemption or if their original request will suffice for this additional requirement. For individuals seeking an exemption from the booster requirement, medical exemptions require a letter from the individual’s primary care physician, and religious exemptions require a letter written by the individual. Employees seeking exemptions from the booster requirement should follow the instructions on the Office of Institutional Diversity and Equity website

If you cannot attend this clinic, vaccine clinics are held every Tuesday and Thursday in January at the former JCPenney location, in the Upper Valley Plaza, West Lebanon. Same-day COVID-19 vaccine and booster appointments are also typically available at Alice Peck Day Memorial Hospital in Lebanon.
