Dear students,

Please send a PDF of your COVID-19 booster shot documentation to medical.records@dartmouth.edu if you have not already done so. You are required to share this information by the Jan. 31 booster shot deadline, and the task should take less than a minute.

Acceptable proof of vaccination includes: 

  • An image of your Centers for Disease Control and Prevention (CDC) vaccination card

  • Formal documentation from your health-care provider or national, state, or local department of health

All documentation must include your name, date of birth, vaccine manufacturer, number of doses and date received, and vaccine lot number. 

If you will be eligible for the COVID-19 booster shot by Jan. 31 but have not yet received it, vaccine clinics are held every Tuesday and Thursday in January at the former JCPenney location in the Upper Valley Plaza. For information about local clinics, visit the New Hampshire Vaccine Information website.

 If you will not be eligible for the COVID-19 booster shot by Jan. 31, you are required to receive the booster within 30 days of becoming eligible. Please submit your documentation after you receive the shot. For information on who is eligible for a booster, visit the CDC website

Individuals who have been approved for a medical or religious exemption from the COVID-19 vaccination requirement will be contacted alerting them if they need to reapply for an exemption or letting them know that their original request will suffice for the booster requirement. For individuals seeking an exemption from the booster requirement, a letter from the individual’s primary-care physician is required. Religious exemptions require a letter written by the individual. Students seeking an exemption can send letters to medical.records@dartmouth.edu.
