To all faculty and staff,

All faculty and staff are required to receive a COVID-19 booster shot by Jan. 31, or within 30 days of becoming eligible. Please submit your booster record using this vaccine tracker form. If you have already submitted a record of your initial vaccine series, you only need to submit your booster dose. You do not need to re-enter your initial doses. 

Acceptable proof of vaccination: 

  • An image of your Centers for Disease Control and Prevention (CDC) vaccination card

  • Formal documentation from your health-care provider or national, state, or local department of health.

All documentation must include your name, date of birth, vaccine manufacturer, number of doses and date received, and vaccine lot number.

If you will be eligible for the COVID-19 booster shot by Jan. 31 but have not yet received a booster, you are encouraged to get boosted at the community vaccine clinic held every Tuesday and Thursday in January in the former JCPenney location in the Upper Valley Plaza, West Lebanon. There is no residency or insurance requirement and appointments are not needed. For more information, visit the New Hampshire Vaccine Information website.

If you will not be eligible for the COVID-19 booster shot by Jan. 31, you are required to receive the booster within 30 days of becoming eligible. After you receive your booster shot, please submit your documentation on the vaccine tracker form. For information on who is eligible for a booster, visit the CDC website

 Individuals who were previously approved for a medical or religious exemption from the COVID-19 vaccination requirement will be contacted alerting them if they need to reapply for an exemption or letting them know that their original request will suffice for the booster requirement. For individuals seeking a medical exemption from the booster requirement, a letter from the individual’s primary care physician is required. Religious exemptions require a letter written by the individual. Employees seeking an exemption can follow the instructions on the Office of Institutional Diversity and Equity’s website
